One of the most rewarding parts of my work here at Liberty HealthShare is getting to see members graduate from our HealthTrac program. If you aren’t familiar with HealthTrac, it is a program for those sharing members who enroll in Liberty HealthShare with certain pre-existing conditions that can be improved with lifestyle change. These chronic conditions include diabetes, hypertension, heart disease, high cholesterol, obesity, and tobacco use.
At the added cost of $80 per month, and provided these members are willing to make the changes necessary to achieve a healthier lifestyle, they are then matched up with a health coach who will work with them, setting realistic, tailored objectives that work for their unique situation.
Regular communication and check-ins keep members accountable, helping them to keep their commitment and achieve their goals. Once they meet the benchmarks that have been set for them, they celebrate graduation from the program!
Graduation from HealthTrac signifies that a member has set and met goals that have brought him or her from chronic illness to greater health. The best part is that with improved health comes a sense of great accomplishment because THEY have done the hard work. HealthTrac members are actively living out the commitment they make as Liberty HealthShare members to honor God and their community by caring for their bodies. They understand what it means to pursue health as a lifestyle. It’s an incredibly effective program that’s changing lives all the time.
